Man-made Disasters in a Wake of Tsunami This month, The Fukushima Nuclear Accident Independent Investigation Commission issued its final report on the disaster: It was man-made! Here’s a quote from the report: What must be admitted — very painfully — is that this was a disaster “Made in Japan.” Its fundamental causes are to be found in the ingrained conventions of Japanese culture: our reflexive obedience; our reluctance to question authority; our devotion to ‘sticking with the program”; our groupism; and our insularity. Had other Japanese been in the shoes of those who bear responsibility for this accident, the result may well have been the same. The last sentence is particular insightful — the blame was not rested on the shoulders of a particular individual, as tempting as that might be, or even on the shoulders of some manager. There’s been a shift in our culture (at least in US) towards seeing medication as a sign of weakness from one of alleviation of suffering that predominated out society some 100 years ago. Some people I know are even proud of the fact that they’ve never taken a painkiller or were treated for cough. Stoicism became a virtue all in itself — “I’m a good person because I don’t take medicine, preferring to suffer the illness and/or the symptoms of the disease.” And it’s not just the patients that feel this way. Medical professionals routinely prescribe to the “complain 3 times” rule: their patients have to mention being in pain on multiple visits prior to getting a prescription that would deal with it. A friend told a story of a doctor visit during which he was told that “he didn’t want to appear to be complainer.” Several weeks later, he was having back surgery and remains in a wheelchair to this day, a decade later! In my book on product design, Interfaces.com, I talk about a shift from evolutionary product design to the current model of version numbering. There has been a murmur of disappointment this last week when Apple issued iPhone 4S instead of the expected iPhone 5. The new features of iPhone 4S are exciting, new, and unexpected (think Siri). So what’s the problem? Is it really that the version number is too low for our expectations? In the days of yore, no one has ever heard of version numbers. What hammer do you own? Is it version 1.0 or 5.0? Do you care? We have many different types of hammers in our garage, but we don’t think of them in terms of version numbers, rather we focus on what they were designed to help us do: hammer a nail in the wall; pull out a nail; tack in the a little staple in the floor boards; create a hammered copper art piece. The Cultural Context for Product Design

Nothing exists in isolation. Design divorced from the context in which the product is used is of little value to its audience. Cognitively, this makes sense—most designers agree that they have to consider the environment, culture, and situation as part of the process of developing a new product (or redesigning an old one). But practically, context and culture get little play in design meetings. Lost in Translation: Cultural Differences in Advertising

“Lost in Translation” was a wonderful movie by Sofia Coppola, starring Bill Murray and Scarlett Johansson. It depicted the delicious confusion of Western tourists in total Japanese cultural emersion. In particular, the scenes where Bill Murray shoots a liquor commercial for the Japanese market are simply priceless. In retrospect, I see where Ms. Coppola got her ideas. Her cousin, Nicholas Cage, have been making wonderfully odd (to our sensibilities) commercials for years. He clearly had stories to share. This morning, my husband and I went to a local grocery store in Rome, Italy. In the cheese section, there was a little paper bottle of parmesan cheese with a mouse of the package. Information Awareness & Failure Analysis

Given the current state of affairs in Japan’s nuclear facilities, I thought it would be good to do a quick analysis of what’s going wrong and why the officials on the ground act as they do (based on very limited information that’s trickling in via the news sources). As of today (morning of March 14th), we have two reactors that have experienced explosions, partial core meltdowns, and multiple other failures. I’ve put together data from the news with failure analysis for an alternative view of the ongoing nuclear crisis in Japan. Like many aspects of usability, FAA (Federal Aviation Administration) was the first to develop practical understanding of Information Awareness and Failure Analysis—pilots and airplane designers what to minimize errors in flight and understand failure when it happens. Like the rest of the world, I’m extremely grateful for their insight into these two aspects of systems design and usability. Below is a quick introduction to the basics.
